HSC knows that it’s not easy to change lifelong habits, no matter what the benefits. That’s why participants choose one area they wish to improve upon and work with a brain health coach to meet their personal goals. Through online surveys, participants get personalized feedback as they chart their progress toward better brain health.

The Self-Management Program for Brain Health will support participants to make lifestyle changes that will promote brain health and cognitive functioning. A combination of health coaching, targeted assessment with feedback, and education will be used to provide a personalized approach to addressing brain function and health, empowering participants to self-manage their participation.
