Family Medicine Clinic

 

What are your hours?
8 am until 5 pm / Monday through Friday. Phones are answered from 8 AM until 5 PM.
* Our answering service is available after hours.

 

Where are you located?
Each of our family medicine clinics is located throughout the Metroplex.

 

Where should I park?
Each clinic has designated free parking for their patients. At the Family Medicine Clinic, patient parking is free in ‘Lot C’ just behind the Health Pavilion. Park at any blue curb (marked ‘clinic’) and enter the building on the north side.

 

What hospitals are you affiliated with?
Medical City Fort Worth; plus, many of our faculty physicians have privileges at other area hospitals.

 

What payment methods do you accept?
We accept cash, checks, and Visa / MasterCard. We participate with most HMO, PPO and commercial insurance programs. We accept Medicaid, Medicare and CHAMPUS assignments.

 

Do you file my insurance, or do I have to?
As a courtesy, we will file your claims on your behalf.

 

Do you charge for a no show?
Failure to cancel an appointment is discourteous. Please call the clinic if you will not be able to keep an appointment so that another patient can be scheduled. We do appreciate you calling as far in advance as possible if you must cancel an appointment.

 

How are emergencies handled after hours?
Nurses are available to address inquiries during normal business hours.
Calls are forwarded to a 24-hour answering service.

 

Do you have a wheelchair to help me from my car to the office or vice versa?
Assistance is provided upon request.

 

Is there anyone in the office that is bi-lingual or multi-lingual?
Interpreters are available upon request.

 

What is the age range of the patients you see?
We specialize in total healthcare for the entire family.

 

Are you seeing new patients?
Yes, we are accepting new patients.

 

How soon can I be seen?
Scheduling varies according to each clinic. Please contact the individual clinic to schedule an appointment.

 

What is your policy for releasing records?
All medical records are property of the clinic. If medical records need to be released, we will ask you to sign a written request for these records to be copied and mailed to a requested party
