“Through our remote patient monitoring program, I have seen our patients gain a better understanding of their health status and become empowered to make changes for the better. They can take ownership of their health by tracking their vitals daily and receiving instant feedback from nurses and providers. It has also enabled my patient’s family members to be able to track their loved one’s health status, even from a distance.”
