As an Acting Systems Redesign Coordinator I help facilitate integrated healthcare teams through the Lean A3 process improvement framework in order to develop, coordinate and implement newly redesigned processes that are created by front line employees. I work closely with the executive leadership team, as well as other high-level management personnel to analyze management techniques, processes, and styles for improving organizational effectiveness. I assist with analyzing the facility, strategic business units, division and service multi-functional programs, functions, and outcomes to determine whether the management systems in current use efficiently accomplishes objectives sought. With the help of the improvement teams I monitor the collection and analysis of organizational outcome data related to the delivery of patient care to determine if the implemented process improvements are producing the desired outcomes. I facilitate Rapid Process Improvement Events (RPIE) utilizing the Lean A3 process improvement framework and coach RPIE teams during weekly completion plan calls in order to execute long term improvement plans, including piloting standard work and re-assessing key metrics. I provide training on Lean A3 thinking and coach participants on how to facilitate projects and Rapid Process Improvement Events. Additionally, I have provided coaching to service areas on how to utilize Huddle Boards for continuous improvements within their work areas. I am also responsible for the event coordination of our monthly Journey of Excellence Lean Celebrations, which highlight the RPIE teams and the work that is accomplished.

Characteristics of high-risk patients include duo-eligible Medicaid and Medicare placement, high re-hospitalization rates, high emergency department utilization, and lower health literacy. Patient-centered plans created with clients to address significant barriers to health and establish SMART goals to increase self-efficacy. Maintain interdisciplinary communication between medical provider, behavioral health specialist, dental provider, enrollment specialist, and other clinic staff to address patients’ physical, social, psychological, and spiritual needs. As a patient health educator, my responsibilities include developing and implementing interventions for a variety of patient needs including tobacco cessation, diabetes management, weight management, and nutritional guidance. Through the use of motivational interviewing and patient-lead consultations, my patients are able to focus on small changes that will eventually lead to a better quality of life.
