The physical therapist visits the patient in their home within the first week of hospital discharge to perform a thorough fall risk assessment and home safety evaluation.

Based on the individual findings and needs established in the initial physical therapy evaluation, the physical therapist makes home safety recommendations, prescribes individualized home exercises, and follows up weekly for in-home physical therapy treatments

An important goal in the STEP physical therapy program is to prevent falls, improve balance, and maximize safety and confidence in the home and community so that the individual may enjoy a better life.
