Q.  Who do I call for spills and/or emergency services?

A.  Call  extension 2181; the custodial staff is notified immediately.

Q.  What is the process of requesting floor work (carpets cleaned, floors stripped)?

A.  Electronically: Use our online Work Order Request.


KEYS

Q.  What is required for Lost Keys?

A.    There is a $10.00 fee for each lost key, and another key request form must be issued before replacement.

Q.   Who do we contact for damaged keys?

A.    The locksmith may be contacted directly at extension 0112.  For replacement the damaged key must be returned to locksmith at the time new key is replaced.

For keys broken in lock an electronic work order must be electronically submitted through Archibus.

Q.  Emergency Request for broken key in the lock?

A.    The locksmith may be contacted directly at ext. 0112.

Q.  For Lock Out?

A.    The Police Department must be contacted at ext. 2110.

Q.  Issuance of Keys

A.    A key request form must be filled out and submitted to Campus Police, once appropriate paper work has been signed off on, the locksmith prepares the key for the request. Normal turnaround time for a key request is two days once work order is received. Keys are picked up at the Police Department.


LIGHTING

Q.  Who do I call for light replacement?

A.   Electronically:  Use our electronic online Work Order Request in Archibus.


PLUMBING

Q.   Emergency Calls:  (Which includes broken lines, gas leaks, flood health, safety?)

A.    Call extension 2181.

Q.   Non-Emergency Calls(Which includes Stopped up toilets, drinking fountains, leaking or dripping faucets, eyes washes, emergency showers, commodes)?

A.    Electronically:  Use our electronic online Work Order Request Archibus.

Q.  Installations: (New Equipment: airlines, gas lines, fume hoods, relocation lines)?

A.    Electronically: Use our electronic online Work Order Request Archibus.  Please allow five (5) working days to process once request is received.


TEMPERATURE

Q.  What to do if room is  too hot or too cold?

A. Call 0465 if you are having temperature problems.

Q. Can I use a space heater at my desk or in my office?

A. Space heaters are not allowed anywhere on campus. They are a fire hazard and waste energy that costs the University money.


FACILITIES MANAGEMENT SERVICES

Q.  What to do for table /chair set-ups?

A.     Electronically submit a Work Order Request Archibus Please allow five (5) working days for set-up.  A diagram must be attached to the work order.  For help attaching a document please call 2161.

Q.  What to do for moving of office equipment?

A.    Electronically submit a Work Order Request Archibus. Please allow five (5) working days for set-up.  An ETR or ETN must be faxed to extension 0300 when request is submitted.

Q.  What to do for rearranging of furniture?

A.    Electronically submit a Work Order Request.  Please allow five (5) working days for moves.

Q.  Who handles repairs to carpet tiles, floor tiles, pictures, doors signs, etc.?

A.    Electronically submit a Work Order Request Archibus  Please allow five (5) working days for scheduling.

FACILITIES STATE VECHILE USAGE

Q.  How do I reserve a vehicle for department usage?

A.    Fill out a work order request for date and times a vehicle is needed.

Q.  How do I get permission to drive a school vehicle?

A.    Contact Mary Solorio at Facilities Management at extension 2184 for the paperwork needed for approval to drive.

Q.  How long does it take to get a driver approved for driving?

A.      After all forms are turned in, it takes approximately 2-4 hours for approval.

Q.  Who do I contact regarding driver approvals?

A.         Contact Mary Solorio at extension 2184 in Facilities Management.

Q.  Where do I pick up keys to the vehicle I have reserved?

A.       Keys can be picked up from the Facilities Management Building.  If you need the keys for a weekend or evening event, keys may be picked up and returned at the Campus Police.

Q.  Do I record mileage for my trip?

A.       Yes – There is a form in each van to record your beginning and ending mileage.
