Refer Family, Friends, and Patients for Dementia Support!

HSC Health’s Center for Older Adults has been selected by the U.S. Centers for Medicare & Medicaid Services to participate in the Guiding an Improved Dementia Experience (GUIDE) Model, a program designed to enhance care for individuals living with dementia and provide vital support for their caregivers.

Through this initiative, our expert team — comprised of geriatricians, neurologists, pharmacists, social workers, and health coaches — coordinates personalized care to help patients remain safely at home. Eligible participants must have traditional Medicare, a formal dementia diagnosis, and not reside in a long-term nursing facility. Caregivers benefit from 24/7 support, education, respite care coordination, and access to community resources. GUIDE program costs are covered by Medicare, and there is no copay for GUIDE-related services.

Additionally, our Dementia CARES class is open to all caregivers, regardless of program eligibility.
