Flexible Work Arrangements and ADA Requests

HSC Campus HR wants to provide you the following information related to Flexible Work Arrangements and the Americans with Disabilities Act so you can remain informed of options that may be available to you.

Flexible Work Arrangements and ADA

Team members who are not required to work in-person, on-campus or who are seeking a flexible schedule must have an approved Flexible Work Arrangement (FWA) form on file with HSC Campus HR and have completed related training. Team members who were previously on an approved FWA must submit a new request if the arrangements are continuing into the new fiscal year. All existing FWAs will expire on August 31st. Additionally, FWAs should be submitted, evaluated and resubmitted at least every six months.

Team members who are seeking reasonable accommodations due to a disability under the Americans with Disabilities Act (ADA) or an FWA should follow the established System and campus procedures outlined in the links below.

We have streamlined the process for requesting a FWA, and the new form is a Dynamic Form which will route for the appropriate signatures automatically, with HSC Campus HR being the final level of review.

Training

Supervisors and team members must complete the training on FWAs, regardless if completed previously.

If you are a supervisor with a team member that requests an FWA and you will also request an FWA for yourself, you must complete both the Employee and the Supervisor training. Secondary supervisors are also required to complete the Supervisor training.

If you have any questions, please talk with your Campus HR Business Partner and/or your supervisor and resubmit FWA documentation or seek a reasonable accommodation, as applicable.

  • COVID-19 Testing is available on-campus by appointment (817-735-5051) during regular operating hours.

– Monday and Thursday, 7 a.m. – 4 p.m.
– Tuesday, Wednesday, and Friday, 8 a.m. – 5 p.m.

  • An on-campus COVID-19 vaccine clinic is scheduled in the HSC Health Pavilion on Friday, August 6 from 8:30 a.m. – 11:30 a.m. To schedule your appointment for a vaccine, please call 817-735-2273.
