Centralized Reporting Tool for Care Team Referrals, Violations of the Student Code of Conduct, and Reports of Sexual Assault/Misconduct

 

The HSC cares about our students’ success, not only academically, but emotionally and physically as well. The HSC also strives to provide all students with a campus environment that is conducive to academic endeavors and social and individual growth. Because of our commitment, we strive to provide services across campus that respond to our students’ unique needs. In an effort to increase opportunities to identify students of concern, a centralized reporting tool was created to capture Care Team referrals, violations of the Student Code of Conduct & Discipline, and Sexual Assault/Sexual Misconduct (Title IX) reports within one online form. The Person of Concern Reporting Form can be found at: http://unthsc.edu/personofconcern. Individuals making a referral can select the nature of their report and identify and provide context for the nature of their concern. By utilizing one central referral method, opportunities are created to gather complete and thorough information in an effort to support our campus in a holistic manner.

The Care Team is designed to support students through non-clinical case management by connecting them with resources and serving as an on-campus source of support.

How to Refer a Student to the Care Team
The Care Team serves a centralized reporting source for any individual (students, faculty, staff, parents, etc.) who is concerned about a student and would like assistance. Referrals can be made through the Person of Concern Reporting Form found at http://unthsc.edu/personofconcern. You can select the nature of the report and identify and provide context for the concern being reported.
