Volunteers Needed!

Participate in a Research Study on:

The Impact of Medical records versus no Medical Records on Medication Therapy Management (MTM) provided by community pharmacists

You must be 18 years or older, African American, and diagnosed with diabetes or high blood pressure

About every 3 months for 2 years, you will:

  • Meet / talk with a pharmacist about your health
  • Undertake a few finger stick blood tests
  • Complete a surveysabout you, your health and your community

The first visit of the study takes place at:
