All new membership forms must be turned in between 8 am-5 pm, Monday-Friday. If this time frame does not work for you, please e-mail Recreation@unthsc.edu for more information.

• Fitness Center Usage Waiver and Standards of Conduct (for ALL memberships)
• Payroll Deduct Form (for UNTHSC employees, Also used to terminate membership)

Employees: Please be sure to routinely check your pay stub to ensure the proper fees are being deducted. The fitness center does not provide refunds under any circumstances.

UNTHSC Child Policy
No one under the age of 16 is allowed in the fitness center under any circumstances. (Includes use of outdoor basketball court)
