GSBS AND SCHOOL OF PUBLIC HEALTH STUDENTS:  Summer Health Insurance Waiver and Enrollment is now open. 

 

TCOM and SCHOOL OF HEALTH PROFESSIONS STUDENTS: CHECK BACK IN LATE JUNE 2012 FOR FALL 2012 ENROLLMENT DATES.

 

If you have comparable insurance coverage through another source and do not wish to enroll in the United Healthcare Student Resources coverage, please complete the waiver form.

If you wish to enroll in the United Healthcare Student Resources coverage for the summer semester

*Students who find themselves without coverage due to age limitations or employment changes should contact Lee Tayon in the office of student affairs to obtain insurance coverage prior to the next regular enrollment period.


 

ALL students will be charged for the UNT System health insurance coverage underwritten by United Healthcare Student Resources unless you have insurance coverage from another health insurance carrier that meets the minimum qualifications set forth by the institution. The policy on student health and minimum health insurance qualifications can be viewed on the Student Policies website. 

Instructions on how to provide a waiver if you have other coverage can be obtained by viewing the PDF training files below. During the appropriate enrollment period you may log on to United Healthcare Student Resources to complete the waiver or enroll in coverage. NOTE: Enrolling is recommended so that insurance cards can be mailed without undue delay.

TEXAS COLLEGE OF OSTEOPATHIC MEDICINE AND SCHOOL OF HEALTH PROFESSIONS
Students enrolled in the Texas College of Osteopathic Medicine and School of Health Professions are required to enroll in an annual plan or waive out with the understanding that their insurance must be in effect for the full school year and meet the minimum comparable coverage guidelines. GRADUATE SCHOOL OF BIOMEDICAL SCIENCES AND SCHOOL OF PUBLIC HEALTH
Students enrolled in the School of Public Health and the Graduate School of Biomedical Sciences will be required to enroll or waive coverage for each term of enrollment with the exception of the summer semester. Alternate insurance must meet the minimum comparable coverage guidelines. Click here to enroll or waive out now.

COMPARABLE COVERAGE SHOULD INCLUDE:

$100,000 individual minimum paid benefit per injury or illness
Individual deductible not to exceed $1000 per policy year
Some level of in-patient and out-patient Mental Health Benefits
Primary Care Services (physician office visits), as well as urgent and emergency room care
Some level of prescription drug coverage 

For International Students (in addition to the above)

Policy written in English
Policy must pay for treatment in the U.S.
Repatriation insurance coverage of at least $10,000
Evacuation insurance coverage of at least $7,500.
**If your plan does not include repatriation and evacuation, but meets all other criteria, a separate policy covering repatriation insurance may be purchased.
